Insurance Coverage

DMTC will bill the insurance company if there is a prescription from your doctor if you were involved in an automobile accident or under workman's compensation. You will have to fill out our paperwork and we will call to your insurance company for pre-approval. 
Most major medical insurance is not covered by massage but because the public is asking for massage coverage check with your insurance company to be sure.

Complete the following forms and return them to DMTC.
We must receive authorization from your insurance company before an appointment can be scheduled.
